Know Your Numbers - Final On-site Screening Dates for 2017

As part of the , benefits-eligible employees are invited to attend an on-site health screening, or have one completed in a primary care physician's office by June 30. By completing a health screening (on-site or with your physician) and the online health risk assessment, employees are eligible to receive $150 added to their pay as a Tier One reward.

Completing Your Tier One Activities

If the health screening is completed on-site with Be Well Solutions, Â鶹´«Ã½'s wellness partner, it will automatically be loaded into your personal portal. Employees who wish to have their health screening completed in a primary care physician's office must log in to the Be Well portal, under "My Wellness," select "Wellness Reward Program," print the "Form 1-Physician Screening Form" and take it to the physician’s appointment. Completed forms must be returned to Be Well Solutions using the address on the form.

Please note that all Tier One requirements MUST be completed by June 30. Employees may be working on their Tier Two activities simultaneously with Tier One, but they will not be eligible for the Tier Two $150 incentive until Tier One requirements are complete.

  • Employees should login to their private Be Well portal at . Those who have never logged into Be Well can get special instructions for first-time access here.
  • Once logged in, select "My Appointments," then "Worksite Screening." Choose a date and location. Please keep in mind that participants will need to fast for eight to 10 hours prior to the health screening and blood draw, which is obtained via a finger prick.
  • Employees can complete the online health risk assessment while logged into Be Well by selecting "My Wellness" and then "Health Risk Assessment."

Once the health screening and health risk assessment are complete, the Tier One requirements are satisfied. Kent State Employee Wellness will be notified by Be Well and the $150 reward will be processed within two pay cycles after notification by Be Well to Kent State. All screening results remain confidential at all times.
